A full-time PA / NP position is available with Liberty Healthcare's medical group in Delaware.
The incoming Physician Assistant or Nurse Practitioner will join two experienced Nurse Practitioners and a Medical Director / Geriatrician.
This group will share responsibilities for providing primary care services to residents of the Delaware Hospital for the Chronically Ill (DHCI).
DHCI is a nursing home, with both skilled and intermediate care beds, which is funded and operated by the State of Delaware.
Approximately 110 older adults and permanently disabled individuals reside at DHCI.
The nursing home is located in Smyrna, Delaware - 45 minutes south of Wilmington.
As a member of Liberty's group, your responsibilities will include:
Providing primary care services to a caseload of approximately 50 assigned patients Conducting examinations, performing diagnostic tests, providing preventive care, developing and managing treatment plans for acute and chronic diseases, and prescribing medications Providing consultation and health education to ancillary staff as well as patients' families and guardians Collaborating with the facility's medical and ancillary staff and actively participating in multidisciplinary treatment team meetingsAs a full-time employee of Liberty Healthcare you can expect:
